- We give continued support to the bereaved family and when necessary family members are referred to our Bereavement Councilor.
How the service operates
The home care team offices are in Mandeville House, The Quay, Clonmel. Here the consultant, registrar, nurses and occupational therapist hold the multidisciplinary team meetings and which is the permanent base for the nursing team.
Logistics and all business related functions are performed by the Operations Manager who also acts as the contact person for the various support groups and fundraisers for the Movement. The above mentioned is achieved with the invaluable and continued support of the voluntary Committee members.

Referrals to the STHM originate from family GPs and Consultants from within our area..

Referrals to the STHM originate from family GPs and Consultants from within our area. Occassionally, they are already receiving care from a palliative care team in hospital outside our area where they are being treated and on return home or to a local hospital this care is continued following consultation with the patient's own Family Doctor and Clinical Nurse Specialist in all hospitals.
The Palliative Care Team works closely with General Practitioners, Public Health Nurses and Hospital Consultants to provide a comprehensive and effective service.
Our services are designed for those with a life threatening illness, who meet the criteria for requiring specialist palliative care. STHM provides a seven day service and we are available on a 24 hour basis for telephone support or visits, if needed. Through funding from the Irish Cancer Society, we have been able to offer a night nursing service for a total of ten nights for each patient, if required.

STHM believes in a holistic approach to the problems associated with death and dying for the patient and family. Bereavement and grief are natural experiences and the majority of people find their own way through their experience or loss. Nevertheless some people will experience difficulties and may benefit from the support of someone outside their own family / friend circle, or from a professional counselling service.
The counselor works in the office in Clonmel on a part-time basis providing one to one counseling and a support service on an individual, family or group basis. The aim of this service is to facilitate the grieving process in a safe and secure manner. This counselling is only offered to relatives of patients whom the clinical team deem would best benefit from the service, it is not a universally accessible service.
